Please ensure that you already have completed and have the scanned / digital copies of the below documents before starting to fill up the application.

1. Passport Size Photo of the applicant

2. Completed Spiritual Background and Personal Testimony Form

3. Completed Recommendation Forms - These must be mailed directly by the Reference Provider to admissions.mzbs@gmail.com

i.  Ministerial / Pastor's Reference Form

ii. General Recommendation Form

4. Completed Medical Certificate of Physical Fitness

5. Completed Financial Sponsorship Form

6. Scanned copies of your Degree Certificates and Marklist
